Abstract

Background: COVID-19 is a pandemic that occurs in all parts of the world and can cause worsening health conditions and even death. Several factors are suspected to aggravate risk factors that can worse clinical conditions and cause death in COVID-19 patients. Risk factors such as age, gender, and the presence or absence of chronic disease are often associated with increased mortality in COVID-19 patients. This study aimed to determine the relationship between factors that affect the survival of COVID-19 patients, such as age, gender, diabetes mellitus, hypertension, heart disease, kidney disease, lung disease, smoking history, alcohol history, and obesity. Methods: An analytical description study of 66 participants was conducted for bivariate and multivariate analysis. Results: The results showed that aged more than 45 years(p=0,001; OR 6,111; 95% CI: 2.102-17.769), male (p=0,003; OR 5.417; 95% CI: 1.862-15.756), diabetes mellitus (p=0,000; OR 36.000; 95% CI: 7.194-180.151), heart disease (p=0,003; OR 9.286; 95% CI: 1.894-45.522), kidney disease (p=0,001; OR 8.953; 95% CI: 2.192-33.686) dan lung disease (p=0,002; OR 7.632; 95% CI: 1.943-29.971), smoker (p=0,003; OR 4.846; 95% CI: 1.683-13.955), dan obesity (p=0,005; OR 4.875; 95% CI: 1.600-14.855) influenced the patient's survival. Conclusion: Risk factors from age, gender, diabetes mellitus, heart disease, kidney disease, lung disease, smoking, alcoholism, and obesity influence the survival of confirmed COVID-19 patients. Among these factors, the most influential is diabetes mellitus.

Abstract

Meningitis is an infection of the meninges that has high mortality and morbidity. Magnesium is an enzyme co-factor that plays a role in cellular physiology. Administration of magnesium has a neuroprotective effect that potential to improve the clinical outcome of meningitis patients. An experimental knowing the effectiveness of oral magnesium on clinical outcomes in meningitis patients at RSUP Dr. Mohammad Hoesin General Hospital Palembang and this is a randomized control trial (RCT) using the add-on and double-blind method in meningitis patients by measuring the Bartel Index (BI) and Modified Rankin Scale (MRS) scales at RSMH for the period 1 September 2022 – 30 November 2022. There were 30 meningitis patients, 13 patients (43.3%) were dominated by tuberculous meningitis, and another types of meningitis were bacterial meningitis, cryptococcal/fungal meningitis and viral meningitis for 11 patients (36.7%), 4 patients (13, 3%) and 2 patients (6.7%) with female sex 16 patients (53.3%) and male 14 patients (46.7%). The difference of value between BI and MRS, the magnesium group is better than the placebo group with a comparison value of 49,333 and 45.33 for the Bartel Index and comparison value of 2,867 and 2,533 for the modified rankin scale. However, this difference was not statistically different with p value > 0.05. The conclusion is there was an increase in BI and MRS values in both the placebo and magnesium groups with the increase in BI and MRS values being higher in the magnesium group.

Abstract

The coronavirus disease 2019 (COVID-19) is caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). Due to its heterogeneous disease severity and rapid transmission, the use of antiviral and immunomodulator drugs is crucial. Favipiravir, an antivirus, can treat SARS-CoV-2 due to its action on RNA Genome ACE2 receptor, therefore halting the viral replication. However, Adverse Drug Reactions (ADRs) monitoring of Favipiravir is recommended. The study design is a descriptive cross-sectional approach with prospective sampling. The study was held in May-July 2021 and aimed to determine the adverse effects of Favipiravir in confirmed COVID-19 patients in Dr. Mohammad Hoesin General Hospital Palembang. Adverse events such as gastrointestinal symptoms (nausea, vomiting, and decreased appetite) occurred in 2 of 170 patients (1.18%). Several side effects, such as an elevated liver enzyme, leukopenia, neutropenia, hyperuricemia, and increased triglyceride levels, cannot be assessed due to a lack of laboratory results before and after Favipiravir administration. Other side effects such as skin rash, asthma, rhinitis, nasopharyngeal pain, and oropharyngeal pain, were not found in the study.

Abstract

Background: More than 60% of stroke patients suffered from long-term disability due to motor impairment despite underwent intensive rehabilitative therapies. Non-invasive brain stimulation such as repetitive Transcranial Magnetic Stimulation (rTMS) has been reported in improving motor function after stroke through the mechanism of neuroplasticity. Research related to the use of rTMS as adjunctive therapy in ischemic stroke cases has mostly been carried out in chronic stroke patients and until now only a few studies have assessed the effectivity of rTMS in patients with acute stroke. Therefore, this study aims to assess whether rTMS can improve motor function in patients with acute ischemic stroke. Methods: This research was a randomized controlled trial and carried out acute ischemic stroke patients in Dr. Mohammad Hoesin Hospital Palembang. A total of 22 patients were randomly divided into a control group with standard therapy (11 subjects) and a treatment group with standard therapy and 5 sessions of rTMS (11 subjects). Improvement of motor function was assessed with motor strength using the Medical Research Council (MRC) scale performed before and after the intervention on the four limb segments. Bivariate analysis with Paired T-Test was used to compare the strength before and after the intervention in each group and the Mann Whitney Test used to compare the differences of motor strength between groups. Results: All subjects in both groups completed the study and no adverse effects were found in the rTMS group. The MRC scale after the intervention increased in all segments in both groups, but significant differences were only found in the treatment group (hand grip (p=0.011), shoulder abduction (p=0.001), hip flexion (p=0.001), and toe dorsiflexion (p =0.002)). Comparison of differences in motor strength between the two groups using the Mann Whitney Test revealed that the improvement was more significant in the rTMS group compared to the control group (p=0.024, p=0.031, p=0.016, and p=0.021). The motor strength in the rTMS group start to increase on the second and third day and particularly reached the peak on the fourth day after the procedure. Multivariate analysis showed no effect of confounding factors such as age, sex, BMI, comorbidities, onset, lesion location, NIHSS, and mRS inthe improvement of motor strength. Conclusion: rTMS therapy can improve motor function in acute ischemic stroke patients and can be considered as an adjuvant therapy in assisting the recovery of post-stroke motor impairment.

Abstract

Ovarian tumors account for 30% of all malignancies in the female genital organs and can be benign, borderline, and malignant tumors. According to World Health Organization (WHO) based on the origin of tumor cells are divided into three groups, namely epithelial tumors, cord-stromal sex, and germ cells where the most types are epithelial types. This study aims to determine the clinicopathological characteristics of ovarian tumors carried out in a descriptive observational manner. Samples using secondary data of ovarian tumor patients who came to the Anatomical Pathology Section of Dr. Mohammad Hoesin Hospital Palembang for the 2018-2020 period who met the inclusion criteria using the total sampling technique obtained 325 secondary data all included as research samples. The results of the study found proportional morbidity rate of ovarian tumors based on the type of histopathology most epithelial tumors obtained by 55%. The highest ovarian tumor patients in the age group of 20-34 years are 91 people, the most common type of tumor histopathology is epithelial tumors as many as 179 cases where mucinous tumor most occur at the age of 20-34 years as many as 23 people, the most growth patterns are cystic 140 cases, the most ovarian tumor behavior is malignant tumors as many as 143 cases. Malignant ovarian tumors are most common in the age group of 20-34 and 45-54 years, which is 37 people each. The cystic growth pattern is most prevalent in benign ovarian tumors, which are as many as 87 cases. It can be concluded that the most common ovarian tumors are epithelial tumors. Based on age and growth patterns, most ovarian tumors occur at the age of 20-34 years with cystic growth patterns. The behavior of most ovarian tumors is malignant tumors.
